Diabetes Nation is a non-profit organization whose mission is to inspire healthcare providers to keep pace with the accelerating complexity of treating diabetes, using simple methods to positively influence performance. Long term goals are to:
- Share successful models of diabetes care through physician collaboration;
- Provide educational programs and materials to simplify patient care decisions;
- Demonstrate the importance of electronic based information tracking, at the primary care level, to improve the quality of health care and reduce costs.
The DIME Program® Care Model is Diabetes Nation’s flagship program to improve diabetic patient outcomes initially in Oregon’s primary care centers, and then in primary care centers across the nation.
The DIME Program® Care Model was implemented as a three year pilot program in 2006 at High Lakes Health Care in Bend, OR. Financial support was received from Regence Blue Cross Blue Shield of Oregon for data collection and analysis during year three. At the time of implementation, High Lakes Health Care employed 15 physicians, 2 nurse practitioners, and 2 physician assistants in the specialties of internal medicine, and family practice.

President, Kevin T. Miller D.O., practices at High Lakes Health Care in Bend, Oregon. Kevin is a current board member and President of the Osteopathic Physicians and Surgeons of Oregon. He also served as the president of the American College of Osteopathic Family Physicians of Oregon. Kevin also serves on the Governor's Board of Rural Health
Medical Director, Richard B. Aguilar M.D., has been in private practice for over 20 years. He is a Board Certified Internist and is the Director of Diabetes Care at High Lakes Health Care in Bend, OR. He is a Professional Member of the ADA and a member of the American College of Physicians. Dr. Aguilar is a national lecturer on the diagnosis and management of diabetes mellitus and has given over 100 Continuing Medical Education presentations on this subject. His passion in medicine is the education and preventative management of the complications from diabetes and cardiovascular disease.
Co-Medical Director, Eden M. Miller D.O., practices with High Lakes Health Care in Bend, Oregon. Her practice is family oriented with a subspecialty in diabetes care. Eden is also a national lecturer for diabetes treatment and prevention, serves on several diabetes advisory boards, and is herself a Type 1 diabetic.
